Complex congenital heart disease including an endocardial cushion defect with abnormal mitral and tricuspid apparatus and an abnormal atrium. The main pulmonary trunk gives rise to one branch for the left pulmonary artery, but then a separate distal main branch also originates, supplying the lingula and left upper lobe, indicating separate origins of the left pulmonary artery. A history of repaired tracheoesophageal fistula is also noted.
