CT angiography shows a pseudoaneurysm arising from the left coronary sinus of Valsalva with surrounding soft-tissue phlegmon posterior to the aortic root and communication with the left ventricle. Pathologic correlation confirmed Staph aureus endocarditis, treated with a pericardial patch and valve replacement.
