Mucinous Adenocarcinoma with Lymphatic Carcinomatosis

Imaging findings
Chest x-ray showed an opacity and increased unilateral interstitial markings on the right. CT demonstrated interstitial thickening, partially smooth and partially nodular, in a patient with underlying emphysema. Micronodular consolidation within the right upper lobe, posterior displacement of the fissure, bronchovascular bundle thickening, and features of lymphatic carcinomatosis (septal nodular and partially smooth thickening) were present. Hilar lymph node involvement was also seen.
Key takeaways
Unilateral increased interstitial markings with a focal opacity should raise suspicion for lymphatic carcinomatosis related to lung cancer. This was a classic appearance of mucinous adenocarcinoma with regional lymphatic carcinomatosis.
