In a 61-year-old with tracheal deviation and a questioned aortic contour on radiograph, CT shows a high-riding aortic arch located well above the pulmonary artery, with the post-arch descending aorta returning to its normal expected location -- producing a buckled, elongated appearance without true hemodynamically significant narrowing and without rib notching or collateral formation. A calcified bicuspid aortic valve and mild ascending aortic dilation are also present.
