Imaging findings
Extensive perilymphatic nodular disease was present in the lungs, predominantly in the upper lobes but also in the lower lobes, with larger opacities in the central left upper lobe. Modest intrapulmonary and hilar lymph node disease was noted. PET showed extensive FDG-avid lesions in the chest and skin, corresponding to skin and cheek lesions. Follow-up imaging after therapy showed substantial resolution of the lung disease.
Key takeaways
This case demonstrates an unusual presentation of diffuse large B-cell lymphoma (DLBCL) with extensive pulmonary involvement, mimicking sarcoidosis or inorganic dust pneumoconiosis due to its perilymphatic distribution. The presence of extrathoracic lesions (skin, cheek) and the dramatic response to lymphoma therapy are key features.
