Abnormal echo led to cardiac MRI showing increased soft tissue along the anterior papillary muscle, iso-intense on T1-weighted sequences, with T2 hyperintensity (possibly flow-related). Post-contrast images demonstrated delayed enhancement engulfing the papillary muscle.
