A patient with cholangiocarcinoma showed thinning of the left ventricular apical wall and septum, indicative of a left ventricular infarct. Delayed phase CT (pancreas protocol) clearly demonstrated a persistent filling defect consistent with an apical thrombus, which was missed on a subsequent echocardiogram.
