In an infant with a repaired VSD, CT imaging revealed an unusual orientation of the minor fissure where the lower lobe appeared to have hyperinflated and slid in front of the middle lobe, displacing it. There was no evidence of lung torsion, but the middle lobe was compressed and displaced by the hyperinflated lower lobe segments.
