Imaging findings
CTA reveals a new, soft tissue mass with fairly uniform intermediate attenuation located under the left breast implant. Associated new axillary lymph nodes are also present. PET CT demonstrates metabolic activity in the mass (SUV 4) and even higher activity in the lymph nodes. The mass appeared somewhat encapsulated on imaging.
Key takeaways
Mammary fibromatosis (desmoid tumor) should be considered in the differential for a soft tissue mass, especially in patients with breast implants or a history of surgery, or those with a predisposition to keloids. These lesions can be hypermetabolic on PET CT and may present with associated lymphadenopathy, raising concern for malignancy. They are often infiltrative and have a high recurrence rate, making surgical management challenging.
