Imaging findings

Extensive mass in the central right lung and mediastinum involving the main pulmonary artery and extending to the left, with absence of normal contrast in the right superior pulmonary vein. Follow-up showed progressive tumor in pulmonary arteries and along the right superior pulmonary vein to the atrium. Ill-defined opacities, ground-glass opacities, peribronchial fluid cuffs, subpleural interstitial edema, and septal lines in the right upper lobe, consistent with pulmonary hemorrhage and tissue infarction.

Key takeaways

Pulmonary artery sarcoma is a rare, aggressive tumor that can infiltrate extensively, including pulmonary veins, leading to venous occlusion, interstitial edema, and pulmonary infarction/hemorrhage. Biopsy may initially show spindle cells, requiring further discussion for diagnosis.
