Truncus Arteriosus (Type II) with Unilateral Left Pulmonary Artery Atherosclerosis

Imaging findings
CT angiography reveals an uncorrected truncus arteriosus (likely type II given the separate, close-proximity origins of the pulmonary arteries) associated with a ventricular septal defect. There is marked asymmetric atherosclerotic calcification and in situ thrombus isolated to the left pulmonary artery, while the right pulmonary artery is larger, free of atherosclerosis, and stenotic at its origin. Prominent systemic-to-pulmonary collateral vessels are visible, predominantly involving the left lung.
Key takeaways
Severe stenosis at the origin of a branch pulmonary artery can protect that vascular bed from the high pressures and turbulent flow of a common truncus, preventing the development of severe pulmonary atherosclerosis that occurs in the unprotected, high-flow contralateral pulmonary artery.
