Chest radiograph demonstrates a steep, rounded elevation of the right hemidiaphragm, particularly prominent anteriorly and posteriorly. Chest CT shows colonic interposition beneath the elevated right hemidiaphragm. The muscular diaphragm is extremely thin at the dome, but the right and left diaphragmatic crura are hypertrophied in an obese patient with a BMI of 36, without signs of crus atrophy.
