Imaging findings
A large opacity is seen on the right side projecting over the central portion of the hemithorax. There is deviation of the trachea to the right, and surgical clips are present in the low neck. CT reveals an absolutely massive amount of thyroid tissue in the mediastinum, growing and insinuating itself between vessels, taking the path of least resistance. The attenuation characteristics, location, and origin are very consistent with a massive goiter.
