Frontal and lateral chest radiographs show a dense, well-defined mass in the right cardiophrenic angle. Chest CT and sagittal reconstructions reveal an anterior diaphragmatic defect containing herniated omental fat as well as a large amount of loculated ascitic fluid tracking into the thoracic hernia sac from the abdomen.
