Ruptured Sinus of Valsalva Aneurysm

Imaging findings
A triple rule-out CT study showed normal coronary arteries and pulmonary arteries. However, a dilated posterior (non-coronary) sinus extended into the right atrium. The right atrium and right heart were enlarged. Phase contrast imaging more clearly demonstrated an aneurysm outpouching into the right atrium. Within this aneurysm, a high-velocity black jet indicated a focal rupture, with most of the aneurysm being contained.
Key takeaways
The patient presented with sudden onset of chest pain and shortness of breath, consistent with acute heart failure. The diagnosis was a ruptured sinus of Valsalva aneurysm from the non-coronary sinus, with flow into the right atrium and right ventricle. Most sinus of Valsalva aneurysms are congenital and can dilate over time. Acute rupture into a cardiac chamber can cause sudden hemodynamic compromise.
