Imaging findings
A large, heterogeneous mediastinal mass with peripheral nodular enhancement, internal necrosis, and intralesional air lucencies post-biopsy. Imaging also showed significant vascularity, connection to the pericardium, and an associated pericardial effusion. Pathological examination revealed pleomorphic cellular proliferation of enlarged hyperchromic cells with irregular nuclear contrast, multinucleated/multilobulated cells, collagenous stroma, and mitotic figures, with focal necrosis. Extensive immunohistochemical staining for various neoplasms (including vascular, smooth muscle, hematopoietic, melanoma, carcinoma, lymphoma, and others) was negative, leading to a diagnosis of undifferentiated aggressive neoplasm.
