Mucinous Adenocarcinoma with Extensive Endobronchial Spread

Imaging findings
CT showed significant mediastinal lymphadenopathy in prevascular, paratracheal, subcarinal, and AP window stations. There was a large consolidation in the right upper lobe, and extensive endobronchial spread of tumor (tumoroids) into the small airways throughout the lungs, notably not lymphatic carcinomatosis. Suspected contralateral endobronchial or hematogenous spread was also seen. PET scan revealed hypermetabolic activity corresponding to these lesions.
