Obstructing Primary Lung Cancer Causing Right Middle Lobe Atelectasis

Imaging findings
Frontal and lateral chest radiographs showed an opacity in the right middle lobe, clearly bounded by the interlobar fissures. The lateral projection indicated that the process primarily involved the lateral segment, with the apex of the triangular opacity extending to the hilum. CT confirmed atelectasis predominantly of the lateral segment of the right middle lobe, with a patent medial segment. An obstructing tumor was identified deep within the lobe, taking off the lateral segmental bronchus and pushing upon the medial segmental bronchus. A PET scan showed a hypermetabolic central mass.
