Chest radiograph shows irregular, abnormal pulmonary vessels and cardiomegaly. CT shows mosaic perfusion and right ventricular hypertrophy. Pulmonary endarterectomy retrieved very little central clot, confirming type 3 disease (predominantly small vessel disease).
