Recurrent Hypersensitivity Pneumonitis (Hot Tub Lung) Post-Lung Transplant
Imaging findings
High-resolution chest CT in a bilateral lung transplant recipient shows progressive ground-glass opacities, centrilobular nodules, and expiratory air trapping. The patient's native lungs had been explanted for chronic hypersensitivity pneumonitis secondary to Mycobacterium avium complex (MAC) from hot tub exposure. Bronchoscopy of the transplanted lung demonstrated non-necrotizing granulomas and organizing pneumonia, consistent with recurrent hypersensitivity pneumonitis following use of a new "swim spa."
