Imaging findings
The heart is significantly shifted to the right, with hypoplasia of the right lung (small right bronchial bud and localized parenchyma). Extensive soft tissue in the anterior mediastinum, pushing structures. Multiple soft-tissue nodules and thickening are seen in the pleura, encasing the right lung and causing compression of the trachea. Abdominal CT shows surgical clips and bony lesions, suggestive of a resected primary tumor.
Key takeaways
Extraskeletal Ewing sarcoma, a soft tissue sarcoma, can metastasize extensively to the pleura and mediastinum, leading to significant mass effect, mediastinal widening, and associated lung hypoplasia. It can mimic lymphoma or thymic tumors.
