CT shows a very large, aggressive-appearing anterior mediastinal soft tissue mass interspersed with fat, extending into the left chest. Surprisingly, there was no pleural seeding or metastases despite the massive size. The mass was treated with radiation and chemotherapy followed by extrapleural pneumonectomy, confirming a Type B thymoma.
