A smoker presented with an ugly, necrotic lung cancer. Adjacent to it was a pulmonary hamartoma or mesenchymal tumor, characterized by macroscopic fat and chunky calcium. PET imaging demonstrated typical FDG non-avidity in the hamartoma, appearing photopenic, while the adjacent tumor was hypermetabolic.
