Imaging findings
Chest CT shows diffuse, homogeneous ground-glass opacities throughout both lungs with relative sparing of the subpleural spaces and very faint, patchy perilobular bands of consolidation. Surgical lung biopsy confirmed cellular interstitial pneumonitis consistent with cellular NSIP, alongside focal, patchy organizing pneumonia.
Key takeaways
Cellular NSIP is characterized radiographically by diffuse, symmetric ground-glass opacities with subpleural sparing and minimal or absent fibrosis. Pathologically and radiographically, there can be a spectrum of overlap between NSIP and organizing pneumonia, representing a common response to connective tissue disease or idiopathic etiology.
