Imaging findings

Initial radiographs showed a history of arthroplasty in the right shoulder due to metastatic sarcoma. Later images revealed extensive abnormality in the right first costoclavicular space, suspicious for tumor involving adjacent vessels. CT showed tremendous tumor growth into the brachiocephalic vein, extending down into the right atrium and right ventricle, with large tumor emboli growing into the pulmonary arteries bilaterally, particularly on the right side.
